New Patient

Please ensure that your Physician has Faxed us a copy of your referral and your demographic information to our office. 

(Demographic is address/phone/insurance info/referral notes etc.)

• If your Physician has sent us your referral, we will contact you when we have availability or you may call us to discuss availability and scheduling.

• Once you have been approved and scheduled: Please Fill out your New Patient Intake and the associated extremity that applies and complete and bring with you to your first scheduled appointment.  Please arrive 15 min prior to that appointment to allow any additional paperwork to be processed.

Filling out your New patient intake forms takes 15-20 minutes,  doing this ahead of time allows us to get you into our system quickly.   If you do not fill it out prior, you will need to arrive 30-45 min prior to complete all paperwork.

Please bring your New Patient Intake Forms, Insurance card(s), ID, Referral (if given to you) and if you have a list of your medications and/or allergies to your first appointment.

Please note that you will also be responsible at the time of your appointment, for paying any possible co-pay and/or paying to meet your yearly deductible requirement. 

Most insurance companies require a predetermined co-pay amount that is due at the time of your appointment. Or a yearly deductible to be met prior to coverage costs.

During the initial evaluation, your Physical Therapist will evaluate your condition and prepare a customized treatment plan that best suits your rehabilitation goals.

Your Physical Therapist and his/her aides will continue to help you progress and care for you throughout your unique healing process.
